SpaceX's Starship rocket was lost shortly after its eighth test flight from Boca Chica, Texas. The Super Heavy booster successfully returned to base, but the Starship upper stage experienced engine failure and loss of control, leading to its destruction about 20 seconds before completing its ascent. Contact was lost after separation, and the self-destruct mechanism was activated. The FAA had approved the test despite an ongoing investigation.
