Woman unable to stop husband’s suicide in prison ‘due to broken intercom’

Isaiah Adekunle Olugosi died by suicide in his cell at HMP Wormwood Scrubs between March 27 and 28, 2022. His wife attempted to alert prison staff but was unable to do so due to a broken intercom and phone lines. She drove 60 miles, banged on the gate, and used a buzzer, but it didn't work. The coroner, Richard Furniss, found the intercom had been broken for years, leading to her inability to warn staff. The jury ruled her husband's death a suicide and criticized the prison and Ministry of Justice for not fixing the system. The prison has until April 21, 2025, to respond to the report.
