A common medication mistake is estimated to kill 200,000 patients across Europe each year and the cost the NHS in England almost £1billion per annum.



Doctors warn that a common medication mistake, known as "non-adherence" (failure to follow treatment plans), is causing potentially fatal outcomes for millions of people, costing the NHS £930 million annually, and leading to an estimated 200,000 preventable deaths.
