Gross failure in basic care by midwives caused death of Ida Lock, coroner rules

A coroner ruled that the death of Ida Lock at Royal Lancaster Infirmary in 2019 was due to gross failure by midwives to provide basic medical care, leading to a lack of oxygen and brain injury during delivery. Midwives failed to identify a slow fetal heart rate and provide effective resuscitation. The coroner identified missed opportunities, criticized the "wholly incompetent" neonatal resuscitation, and cited a history of failings at the University Hospitals of Morecambe Bay NHS Foundation Trust. Prevention of future deaths reports are being filed with the health trust, the Secretary of State for Health, and the chief coroner due to concerns about clinical governance and the culture at the trust and the national issue of shortcomings in maternity units.
