In Quebec, 679 people died accidentally in the health system in one year, due to incidents such as falls and medication errors. The number of incidents with severe consequences has more than doubled between 2020 and 2024, and the proportion of events leading to death is up. A national mandate has been given to universities to examine the causes of these accidents and propose preventative actions. Staffing shortages and workload are cited as contributing factors.
