HSE apologises to woman whose son killed her partner after being discharged

Fiona Nolan had brought her 19-year-old son Adam to the emergency department of Tallaght University Hospital because he was unwell and had psychotic thoughts



The HSE apologized to Fiona Nolan for failings in the psychiatric care her son, Adam Nolan, received at Tallaght University Hospital in January 2018. Adam, who was exhibiting psychotic thoughts about killing Fiona's partner, Bryan Cassidy, was discharged without follow-up. Two weeks later, Adam killed Bryan Cassidy. Fiona Nolan settled her High Court nervous shock action against the HSE for €150,000. The HSE acknowledged the "deficits in care" and stated they had reviewed the event and implemented key learning.
