‘Potential systemic failure’ may have contributed to soldier’s death, say family

The family of Lance Corporal Joe Spencer, who died during a sniper training exercise in the Scottish Highlands when his rifle discharged, believes a "potential systemic failure" may have contributed to his death. A pre-inquest hearing will examine how a round remained in Spencer's rifle and whether training protocols were followed. A fatal accident inquiry ruled Spencer's failure to properly unload his rifle and holding it improperly contributed to his death, but also cited failures in the training course delivery.
